Why am I receiving this message?
To qualify for Personal Injury Protection (PIP), the state of Michigan requires confirmation of health coverage documents during policy renewals.
You selected Personal Injury Protection coverage for Allowable Expenses (Medical) with a $50,000 limit.
PLEASE NOTE: If you do not complete the following requirements, upon renewal, the PIP coverage will change to Unlimited, causing a significant increase in your premium.
Step one
Review qualifications
- The policyholder must be enrolled in Medicaid.
- The spouse and resident relatives* must have Qualified Health Coverage (see FAQs for more details), Medicaid or coverage under another auto policy with PIP medical coverage.
* A "resident relative" is a relative of either the policyholder or their spouse who lives in the same household.
Step two
Gather required documents
The policyholder must:
- Submit a copy of their Medicaid insurance card.
- Collect and submit health coverage documentation for the individuals listed on the policy.
- Submit one of the following as proof of coverage for a spouse or resident relatives*.
- A copy of their Medicare insurance card
- Proof of Qualified Health Coverage (see FAQs for more details)
- The policy number and carrier name for PIP coverage under another auto policy must be provided to your Allstate® agency
Proof of Qualified Health Coverage
Acquire a letter from their health insurance carrier, employer or provide other documentation with the following information.
- Name of the health insurance company
- Name of each person covered and their date of birth
- Policy and group number
- Coverage effective dates (within the last 12 months)
- Confirmation that the health insurance is considered Qualified Health Coverage

Send requested documents to Allstate

Frequently asked questions
Qualified Health Coverage can be either of the following.
- Health and accident coverage that does not exclude or limit coverage for injuries related to auto accidents and has an annual individual deductible of $6,579 (subject to annual adjustments by the Director).
- Coverage under both Medicare Parts A and B (a Medicare Advantage plan).
Note: Medicaid and health care sharing ministries are NOT considered Qualified Health Coverage.
You are welcome to submit a Selection of Personal Protection Insurance Benefits form at every policy renewal, but it is not required.
This form describes your options and is prefilled with the PIP coverage option you previously selected.
